Following CDC guidelines, Hoag has outlined the following protocols to ensure your safety and our staff safety.
Cardiac rehabilitation is a comprehensive approach to helping individuals with heart disease reach their optimum level of functioning through a program of monitored progressive physical exercise, risk factor modification, education, and support.
A physician referral is required to participate. Many insurance companies reimburse full or partial costs of cardiac rehabilitation. Checking with your insurance carrier is advised prior to starting the program. For further information or to set up a consultation appointment, call 949-764-5594.
Phase I – Inpatient Program
Phase I begins in the hospital after a cardiac event. This phase includes light supervised exercise, cardiac risk assessment and education. Participants are provided with an educational packet by cardiac rehabilitation and hospital nurses. You will learn about risk factors, diet, your medications, sexual activity, exercise and your home care instruction.
Phase II – Outpatient Program
An individually designed exercise program, consisting of one-hour sessions, three times per week, for a twelve-week period is standard for optimal reconditioning benefits. The exercises prescribed in cardiac rehabilitation are designed to help build strength and flexibility. During exercise, the participant’s ECG, heart rate, rhythm, and blood pressure are monitored closely. Conditioning is attained through gradual progressive increases in workload intensities using a variety of exercise equipment including treadmills, bicycles, Nusteps®, and weights. The staff will also guide you in achieving your lifestyle goals.
Included in the cardiac rehabilitation program is an appointment with our registered dietician. The dietician will be calling to schedule this personal appointment.
Many patients feel depression, anxiety, or anger following their cardiac event. We offer assistance with a licensed clinical social worker to help with your emotional needs. Additionally, a cardiac support group, “Heart Matters,” meets monthly to discuss a variety of emotional issues relating to heart disease.
There is an ongoing Cardiovascular Educational Series included. All participants and their families are encouraged to attend. Class topics include Understanding Heart Disease, Heart Healthy Eating, Cardiovascular Exercise & Strength Training, Medications and Stress Reduction. View Heart & Vascular Education Classes
Phase III – Maintenance
Phase III is a semi-independent unmonitored program, which can start after completing Phase II. This is an optional phase of continued supervised exercise and support, scheduled two or three times weekly and should complement a home program.
Who Will Benefit?
Individuals with any of the following cardiac conditions may benefit from the program:
Heart attack (myocardial infarction)
Chest pain (angina pectoris)
Interventional cardiac procedures like coronary angioplasty, percutaneous intervention (PCI), stents, etc.
Coronary artery bypass surgery (CABG)
Valve surgery
Cardiac Transplant
Heart failure (patients who meet certain requirements)
